What we collect and why

• Questions. Your name, how you would like us to reply, the matching email address or phone number, and your message. We use them to answer you.
• Care requests. Who needs support, the town or ZIP code and county, the kinds of help wanted, the preferred schedule and start, your name and contact details, your follow-up preference, and any caregivers you chose to mention. We use them to contact you and arrange a free consultation.
• Transportation requests and professional referrals. The details you enter about the trip or referral and how to reach you. We use them to respond and coordinate next steps.
• Caregiver applications. If you create an applicant account, we collect your account details and the information requested in our hiring documents, including identifiers needed for employment and your electronic signatures. We use them to consider your application and meet employment requirements.
• Technical information. Your IP address and browser details are used to protect our forms from misuse and, when you sign a hiring document, are stored with your signature as evidence of when and how it was signed.

Please do not send medical records, Social Security numbers, insurance details, or other sensitive information through our general forms. We will ask for what we need, in the right way, if we work together.

How we protect it

Form submissions and applications are stored in our own database. Personal details are encrypted at rest. Only staff whose role requires it can see them, and staff must sign in with a password and an authenticator code. Email alerts to our team contain a link only, not what you wrote.

Who we share it with

We do not sell your information or share it for advertising. We use service providers to host this website and deliver email on our behalf, and they may process information only to provide those services. We may disclose information when the law requires it. If you accept analytics cookies, Google receives usage information as described in our Cookie Notice; it does not receive your name, contact details, or anything you type into a form.

Text messages and marketing

Choosing a reply method tells us how to respond to your request. It is not a sign-up for marketing. This website does not send text messages, and we do not add you to marketing lists from these forms.

How long we keep it

We review closed inquiries 24 months after they close and applications that did not lead to employment 24 months after a decision, and delete what we no longer need. Records of people we employ are kept as employment law requires.

Your choices

You can ask to see, correct, or delete the information we hold about you by contacting us at info@realchoicecare.com, (860) 384-6040, or by post to REALCHOICE CARE LLC, 315 Litchfield Rd, New Milford, CT 06776. We may need to confirm your identity first, and we may keep information we are required to retain. You can change your cookie choice at any time using “Cookie preferences” at the foot of every page.

Children

This website is intended for adults. We do not knowingly collect information from children under 13 through it.
